Information to Send – Please Read Carefully

Fill out the form below to begin sending us the information to build your child’s transcript. You can upload .PDF or Microsoft Word or Excel files of transcripts and supporting documents.

EMAIL OPTION: Instead of filling out the form you can email us the following information to cctucker7@gmail.com:

– name, gender, birth date, and address
– a list of all the middle or high school level courses, dates taken, and the grades earned, indicating if the classes were at the honors level, AP, IB, or AICE.
– a list of all dual enrollment courses with grades and dates taken (attach an unofficial transcript)
– a list of all tests taken: PSAT, SAT, ACT and scores achieved (attach score reports)
– a list of all community service hours accumulated
– a list of all junior or varsity level sports teams played on
– a list of all music, dance, theater, or fine arts classes, activities, or presentations with total hours, locations, dates, and grades (if any)
